Needs Assessment - screening and evaluation of residents' needs, goals or conditions
Service Planning - an individualized service plan developed to address the needs and goals identified in the Needs Assessment.
Referrals and Advocacy - develop a network of providers to implement the Service Plan
Counseling - a means of measuring progress toward filling needs and reaching goals
Homeless Prevention Planning - to identify and avoid behaviors that lead to homelessness
Much, much, more such as meals at SROs, security, recreational opportunities, crisis intervention, maintenance, entitlement assistance, budgeting, employment assistance and opportunities
